The Freedom of Information Act 2000 (FOIA) gives people a general right of access to information held by or on behalf of public authorities. Read about how the information we collect abut you is protected.
You can request any information that City and Hackney CCG holds, that does not fall under an exemption.
If the information is your own personal data, then you should make a subject access request under the Data Protection Act (DPA) 1998, and not under the FOI Act.
Your request must be in writing and can be either posted or emailed to City and Hackney CCG:

Can my request be refused?
Our commitment to publish information excludes any information which can legitimately be withheld under the exemptions set out in the NHS Openness Code or the Freedom of Information Act e.g. the protection of commercial interests and personal information under the Data Protection Act 1998. Do I need to pay costs?
The cost of accessing most information will be free, but there may be a charge for some information. The charges will vary according to how the information is made available and the format. We will let you know the cost and charges that will have to be paid in advance. We will not provide printouts of other organisation's websites.
